For Patients Considering a Visit to Our Clinic

Basic consultation agreement

Our clinic follows a step-up approach to treatment and provides services such as timed intercourse treatment, intrauterine insemination (IUI), in vitro fertilization (IVF), and intracytoplasmic sperm injection (ICSI).

Important: Please confirm the following
before your appointment

Appointments are required for both women and men.
Please note that we are unable to provide consultations if you arrive after your scheduled appointment time.
For the first consultation, we provide a detailed explanation of the treatment, so we recommend that couples attend together.
It takes the entire morning, so please make your appointment on a day when you have plenty of time.
If you are breastfeeding, please wean your child and wait until you have had three menstrual periods before making an appointment.
If you delivered by cesarean section, treatment cannot start until one year after delivery. Appointments can be made from 10 months after delivery due to required examinations.
We kindly ask that you do not bring children to your appointment.
Please be considerate of other patients.
Payment is by cash only. Credit cards and QR code payments are not accepted.
The estimated consultation fee for the first visit is approximately ¥10,000 for women and ¥5,000 for men.

Please check the following items required for your first appointment day, and have your interpreter call us to make your first appointment.

Items to Bring for Your First Appointment (Women)

	①My Number Health Insurance Card or eligibility verification certificate
	②Family Register (Koseki Tohon) or Resident Certificate
	③Residence card
	④Records of basal body temperature, menstrual cycle, and marital sexual activity Basal body temperature is the temperature measured under the tongue immediately upon waking using a gynecological thermometer capable of recording to two decimal places. Please bring data for at least the past month.
	⑤Initial Medical Questionnaire Please download the file, fill it out, and bring it with you.
	⑥Basic consultation agreement Please click the image on the left to download the file, sign it, and bring it with you.
	⑦Interpreter Consent Form for Accompanying Individuals Please click the image on the left to download the file, sign it, and bring it with you.
	⑥Medical Information Letter from Your Current Hospital
	⑦Information about Medications, Herbal Medicines, and Supplements Please bring all items that show details of any prescription medications, Kampo (herbal) medicines, supplements, health foods, or nutritional drinks that you are currently taking. Please bring supplements in their original packaging.
